Medications delivered directly to the brain. Doctors may insert a long, thin tube (catheter) through an artery in your groin and thread it to your brain to deliver tPA directly into the area where the stroke is occurring. This is called intra-arterial thrombolysis. The time window for this treatment is somewhat longer than for intravenous tPA, but is still limited.

One of the major causes of elderly falls is hyponatremia, an electrolyte disturbance when the level of sodium in a person's serum drops below 135 mEq/L. Hyponatremia is the most common electrolyte disorder encountered in the elderly patient population. Studies have shown that older patients are more prone to hyponatremia as a result of multiple factors including physiologic changes associated with aging such as decreases in glomerular filtration rate, a tendency for defective sodium conservation, and increased vasopressin activity. Mild hyponatremia ups the risk of fracture in elderly patients because hyponatremia has been shown to cause subtle neurologic impairment that affects gait and attention, similar to that of moderate alcohol intake.[47]


Both of my sons go to this daycare and love it! My youngest spent his baby days with miss jean and ...still seeks her out for cuddles. He is now in the preschool program and loves miss taylor as well. He can’t wait to go downstairs every day for “school”. He loves singing songs, doing circle time and many other activities. She told me the days we needed her would work and we set up a time to come in for a visit. After months of searching for childcare, we felt relieved! Later that night, I get a call from Sherri, telling me, she just had someone call with 2 full time children and she’s taking them instead. I can’t speak to how the program rates as far as care for the children, but as a business owner myself, this was a poor way to treat a potential client. See More

On both occasions I used their service, the caregivers either left minimal or no notes while visiting my elderly parents (no eating, sleeping or bathroom data). After the first visit, I contacted a supervisor at Around the Clock and asked specifically that the caregivers leave notes on eating, sleeping patterns, etc. Instead of improving, on the 2nd visit I only received notes from 1 of 3 caregivers...I paid for 16 hours of caregiving for people who may or may not have even showed up for all I know. Their medical staff is also hit and miss. On my father's post-hospital rehab, one of the 3 nurses missed appts at the house repeatedly...only making 1 of 5 scheduled appointments. After this, I contacted Around the Clock and asked them to send out a more reliable nurse. In the 5 weeks that have passed since, we have received 1 visit from a nurse...nobody makes their scheduled appointments. Is this the care you want for your elderly parents??

Promoting independence in self-care can provide older adults with the capability to maintain independence longer and can leave them with a sense of achievement when they complete a task unaided. Older adults that require assistance with activities of daily living are at a greater risk of losing their independence with self-care tasks as dependent personal behaviours are often met with reinforcement from caregivers.[42] It is important for caregivers to ensure that measures are put into place to preserve and promote function rather than contribute to a decline in status in an older adult that has physical limitations. Caregivers need to be conscious of actions and behaviors that cause older adults to become dependent on them and need to allow older patients to maintain as much independence as possible. Providing information to the older patient on why it is important to perform self-care may allow them to see the benefit in performing self-care independently. If the older adult is able to complete self-care activities on their own, or even if they need supervision, encourage them in their efforts as maintaining independence can provide them with a sense of accomplishment and the ability to maintain independence longer.[43]
Innovative programs. Doctors communicate using digital video cameras, internet telecommunications, robots and other technology. “If a patient does experience the symptoms of a stroke, it is vital to receive appropriate clinical assessment and scanning in hospital early to confirm it is a blockage of a blood vessel. Patients are then referred to our interventional lab in Beaumont Hospital, the main route to thrombectomy. We will do an endovascular procedure, puncturing the artery and groin to pass a tube in the blood vessels up to the neck and head. Through the tube, we place a stent retriever, which pulls the blood clot out, restoring blood flow to the brain tissue.
Given the choice, most older adults would prefer to continue to live in their homes (aging in place).[15] Many elderly people gradually lose functioning ability and require either additional assistance in the home or a move to an eldercare facility.[15] The adult children of these elders often face a difficult challenge in helping their parents make the right choices.[16] Assisted living is one option for the elderly who need assistance with everyday tasks. It costs less than nursing home care but is still considered expensive for most people.[17] Home care services may allow seniors to live in their own home for a longer period of time.

“Mechanical thrombectomy is a treatment used to remove a large blood clot from inside a blood vessel in a patient’s brain,” explains Dr John Thornton. “Before this was developed, Irish patients were receiving clot-busting drugs to help dissolve the clot instead of pulling the clot out but what became evident was that these drugs didn’t work as well when there was a large blockage in the brain. Thrombectomy provides the greatest chance of patient recovery from large blockages which cause the most devastating strokes.”

Geriatric care managers (GCMs) are usually social workers, psychologists, nurses, gerontologists or others with training and experience in various aspects of elder care. Geriatric care managers not only assess a senior’s needs, they can also handle crises (such as an emergency hospitalization), help place the senior in a residential care community, help resolve family conflicts, locate community resources or fill in for family caregivers at doctor’s appointments, among other services.
